What Does a Home Care Visit Look Like? A Day in the Life with Sylvian Care Tonbridge

If you have never arranged home care before, it can be hard to picture exactly what it involves. What does a carer actually do during a visit? How does it feel for the person receiving care? And how does it all come together in practice?

At Sylvian Care Tonbridge & Royal Tunbridge Wells, we believe that transparency is the foundation of trust. In this article, we walk you through a typical day of home care with one of our clients, to give you a clear and honest picture of what to expect.

Meet Margaret

Margaret is 81 years old and lives in her own home in Tonbridge. She has mild dementia and some mobility difficulties following a hip replacement two years ago. Her daughter, who lives nearby, wanted to ensure her mother could remain at home safely and comfortably without her having to give up work.

After a free care assessment with Sylvian Care, a care plan was created specifically for Margaret. She now receives two visits per day from the same carer, Sarah, whom she has come to know and trust.

The Morning Visit (8:00am – 9:00am)

Sarah arrives at Margaret's home at 8:00am. She has her own keysafe code so Margaret does not have to rush to the door. She knocks and calls out as she enters, so Margaret knows exactly who it is.

Together, they go through the morning routine at a pace that suits Margaret. Sarah helps her wash and dress, choosing an outfit Margaret selects herself. Margaret loves a poached egg on toast and a cup of strong tea, and Sarah sits with her to chat while she eats.

During this time, Sarah checks that Margaret's medication is taken and logs the visit on her care notes. She has a quick look around to check the home is safe and tidy, and notices that Margaret seems a little more tired than usual, which she makes a note to report to the care coordinator.

By 9:00am, Margaret is up, fed, and in good spirits. Sarah says goodbye and confirms she will be back that evening.

The Evening Visit (5:30pm – 6:30pm)

Sarah returns in the early evening. She helps Margaret prepare a simple evening meal, assists with any personal care needs, and ensures her medication is taken.

She helps Margaret get settled comfortably for the evening, whether that is watching her favourite programme, doing a puzzle, or just having a conversation.

Before leaving, Sarah checks the home is secure, makes sure Margaret has everything she needs within reach, and logs her notes.

What Happens on Different Days?

Not every day looks the same. On Tuesdays, Sarah takes Margaret to her weekly coffee morning at the local community centre, which is a highlight of her week. On Thursdays, she accompanies her to a GP appointment and helps ensure the doctor's instructions are understood and followed.

When Margaret's daughter goes on holiday, the visit schedule is temporarily increased to three visits per day, providing extra reassurance for the whole family.

The Care Plan Behind the Scenes

Everything Sarah does is guided by Margaret's personalised care plan, which was created with input from Margaret herself, her daughter, and our experienced care coordinator. The plan covers:

·        Specific personal care needs and preferences

·        Medication schedule and any health conditions to monitor

·        Dietary preferences and any restrictions

·        Margaret's daily routine and what matters most to her

·        Emergency contacts and what to do in different situations

The plan is a living document, reviewed regularly and updated whenever Margaret's needs or preferences change.

How Does the Family Stay Informed?

Margaret's daughter has access to regular updates from our care team. Any concerns are flagged promptly. If there is a change in Margaret's health, behaviour, or wellbeing, her daughter is the first to know.

This joined-up approach gives families confidence that their loved one is truly being looked after, not just ticked off a list.

What Makes Sylvian Care Different?

We know that the small things make all the difference. It is the carer who remembers how Margaret likes her tea. It is taking an extra five minutes to listen to a story about the past. It is treating every client with the same dignity and respect you would want for a member of your own family.

Our carers are matched to clients based on personality and interests, not just availability. We offer minimum one-hour visits so there is never a sense of rushing. And we invest in the ongoing training and support of our team so they can deliver consistently excellent care.

Frequently Asked Questions

What is a home care visit?

A home care visit is when a trained carer comes to your loved one's home to assist with personal care, medication, meals, companionship, and other daily tasks. Visits typically last between one and several hours, depending on the care plan.

How long are home care visits with Sylvian Care?

We offer care visits of at least one hour. We do not carry out 15-minute visits, as we believe these do not allow enough time to deliver the quality of care our clients deserve.

Will my loved one always see the same carer?

We aim to provide continuity of care wherever possible. Clients are matched with a primary carer, and any additional carers introduced into the care arrangement will always be familiar faces to the client before they take on regular visits.
