What is palliative care?

Palliative care is specialist support for people living with a serious, long-term or life-limiting health condition.

Its purpose is to help improve quality of life by focusing on comfort, wellbeing and the individual’s wider needs. Palliative care does not necessarily mean that someone is approaching the end of their life. It can begin much earlier and may be provided alongside treatment or other healthcare.

For many people, receiving this support at home means they can remain in familiar surroundings, close to the people, routines and possessions that bring them comfort.

Who may benefit from palliative care?

Palliative care may be appropriate for people living with conditions such as cancer, advanced heart or lung conditions, neurological conditions or other progressive illnesses.

Every situation is different. Some people may need occasional practical support, while others may require more regular care throughout the day or night.

Our role is to understand what matters to the individual and their family, then build support around those needs.

How can we support someone receiving palliative care?

Depending on the individual's care plan, support may include:

  • Personal care and help with everyday routines

  • Assistance with washing, dressing and continence needs

  • Support with eating, drinking and meal preparation

  • Medication prompts or assistance within the agreed care plan

  • Mobility support and help moving safely around the home

  • Companionship and emotional reassurance

  • Help maintaining familiar routines and independence

  • Overnight or more frequent support where required

  • Practical support around the home

  • Communication with family members and relevant healthcare professionals where appropriate

End-of-Life Care

How is End-of-Life Care different?

Palliative care and end-of-life care are closely connected, but they are not exactly the same.

Palliative care can begin at any stage of a serious or life-limiting illness and may continue for months or even years alongside other treatment.

End-of-life care is support provided when someone is approaching the final stage of their life.

At this point, care is particularly focused on keeping the person as comfortable as possible, protecting their dignity, respecting their wishes and supporting the people closest to them.

End-of-Life Care at Home

For many people, home is where they feel safest and most comfortable.

Where it is appropriate and in line with the person's wishes, end-of-life care at home can help them remain surrounded by familiar faces and surroundings during an incredibly important period of their life.

Our Care Professionals provide sensitive, respectful support built entirely around the individual.

This may include:

  • Personal care delivered with dignity and sensitivity

  • Helping someone remain comfortable and settled

  • Support with repositioning, mobility and everyday needs

  • Assistance with eating and drinking where appropriate

  • Medication support within the agreed care plan

  • Overnight care or increased visits as needs change

  • Companionship and reassurance

  • Maintaining familiar routines wherever possible

  • Providing a calm and reassuring presence

  • Supporting family members and helping relieve some of the practical pressure of caring

  • Working alongside district nurses, GPs, hospice teams and other relevant healthcare professionals where required
